How can I prepare for my office visit?
Please kindly bring your medications to all visits. You may choose to make a list if this is more convenient for you.
Please bring your insurance card and be prepared to pay any co-payment required by your insurance plan.
If you happen to arrive more than 15 minutes late to your visit, please note that you may be rescheduled for another time.
What is your cancellation and no-show policy?
If it is necessary to reschedule an appointment, please call us immediately. Unfortunately, please note that a $50 fee will be associated with appointments not kept or not cancelled at least 24 hours in advance of your appointment time. Please contact the office for more specific details.
